Borrower's Checklist

1. Most recent pay stubs from your present employer.
   
   
2. Signed and dated copies of your federal tax returns for the past two years, three if self employed.
   
   
3. W-2 forms for the past two years, or 1099 forms for the past three years.
   
   
4. Bank statements:
   1. checking accounts
   2. savings or share accounts
   3. retirement accounts
   4. miscellaneous income accounts
      
      
5. List of consumer debts:
   1. bank loans - auto, personal, etc.
   2. credit card accounts
   3. retail accounts
      
      
6. Federal and state V.A. loans:
   1. copy of eligibility certificate or copy of DD214 form (separation from active duty)
